Module 1: Anterior Maxillary Distraction (AMD)

Module 2: Le Fort I Level Distraction

Introduction: Hypoplastic midface is still a common occurrence following cleft lip/ palate repair despite our best efforts to minimise its incidence. The primary problem is a smaller than normal and retruded maxillary arch which results in crowding of teeth, Class III malocclusion and an under projected midface.

Though midface advancement is an established technique and plays a seminal role in the management of the cleft midface, it does not directly address size of the maxillary arch. Anterior Maxillary Distraction (AMD) which is a relatively new technique addresses the midface by increasing the size of the maxillary arch, thus allowing the orthodontist to align the teeth without resorting to extraction. It also preserves the grinding surfaces of the teeth.

In severe cases AMD may be combined with other midface advancement techniques to optimise outcome.

Who will it benefit from this workshop: This workshop is aimed at young cleft and craniofacial surgeons, trainees and practising cleft surgeons unfamiliar with these techniques.

  1. Basic understanding of the dynamics of the cleft midface and alveolus
  2. Decision making in the real world
  3. Basic orthodontic preparation
  4. Planning of midface correction including large discrepancies and crowding of teeth
  5. Surgical techniques, mechanics and force vectors of distractors, dealing with relapse
  6. Pitfalls and potential complications

The Course Structure:

  1. Introductory remarks
  2. The Decision-making process
  3. Orthodontic inputs
  4. Step by step demonstration of the surgical techniques and hands-on experience on dry skull models (2 hours 30 minutes)
